The exact fee is calculated automatically by the official Bitcoin client, as recommended by the Bitcoin developers.
 
It is configured to include sufficient fees so that payouts will confirm within 3 blocks.
 
A block is generated every few minutes on average.
 
Most payouts are confirmed on the very next block. Sometimes, although rarely, the official Bitcoin wallet wrongly estimates the fees, and as a result it takes a little bit longer for the payout to confirm. Hopefully this will be improved in future Bitcoin releases.
