There are no special forms, every month from the 1st to the 6th, check the gold spreadsheet and verify if you are listed as passed.
You do not need to cause a fuss, i also have rejected blogs from last month, i have recycled and improved my efforts and tried to be better by re applying this month once again.

The higher the quality, the higher the chance for recognition for reward.
Keep trying.
