A lot of things need to be improved in this, especially the signature code, you need to mention how many members from each rank you will accept, and it would have been better if you had different payment structures for each rank, paying $20 to Jr. member and paying the same amount to a Full member doesn't seem the best thing to me because you could do it cheaper than that for lower ranks.
